Our Programs

Supported Housing & Case Management

Since 1992 Adelante has provided apartments and houses for persons with mental disabilities. Our supported housing program assists mental health clients in securing transitional housing that will eventually lead to independent living. Services include linkage with a comprehensive community support system of case management, mental health rehabilitation, respite, employment and health supports. Individuals with psychiatric disabilities, who have graduated from community resident programs or have been discharged from a psychiatric hospital, may be interviewed for the program. Clients have to exhibit basic skills that will support a high quality of life. Case managers assist in establishing the household, ADL, and budgeting and empowering consumers. They are on call twenty-four hours a day. Bilingual case management is available. 

This program has high attendance and participation of consumers.  Consumers’ interaction and willingness to participate in group discussions and group activities helps them develop their interpersonal skills; learn how to secure support services through our case management and within the community and to learn how to live independently.

Mental Hygiene (Life Skills, Vocational & Drop-In Center)

Adelante offers programs to empower and build skills among the mentally disabled population. Adelante operates a Drop-In for adult mental health consumers. We provide a comfortable environment for consumers to enjoy themselves and the company of others. The main objective is to assist consumers in developing and reestablishing a sense of self-esteem and group affiliation. The Life Skill and Vocational Training programs assist mental health consumers in becoming productive members of their community. Consumers receive self-help and empowerment interventions, community living skill education, academic enrichment and social skill rehabilitation. Bilingual counselors/ instructors and case managers are available.

In the following categories: (1) Consumer self-help and empowerment interventions; (2) Social-leisure time rehabilitation; (3) Field Trips; (4) Informal counseling; (5) Promoting recovery from mental illness; and (6) Reintegration into a meaningful role in community life, vocational training is tailored to the individual’s qualifications and interest. If the program is not available at our site, we refer them to another agency.

The Suffolk County Office of Mental Hygiene evaluates and monitors these programs, however, Adelante has seen visible improvements on the part of the clients in their ability to dress appropriately, communicate, control their behavior, and the development of skills that will assure employment.

Family And Youth Services

Adelante of Suffolk County has focused on addressing the growing needs and concerns of the community by reaching out and providing them with the necessary resources to assist in their development by providing not only community services and support groups, but also by incorporating and encouraging family structure and support.  In most cases our families are hindered from participating due to economic deprivation and language barriers.  The objective is to strengthen and develop community members that will become active leaders in the community by providing them with the necessary tools to achieve their goal.  Our counselors receive extensive educational training on the issues facing the community and accessing resource guides continually so that they can better services clients needs in the following areas; Department of Social Services, Social Security, Domestic Violence, Child and Elderly abuse, Immigration, Discrimination, Housing, Education issues, Veterans Affair and Crisis Intervention.

Our intervention has provided us with the opportunity to impact in a positive manner those in the communities who are at risk and have the ability to excel. During the last fiscal year Adelante served:

The clients were counseled, given help for suitable educational training, including ESL, others were referred to other agencies for other services such as medical, housing services and legal services.

The Suffolk County Youth Bureau evaluates and monitors this program. Monthly progress reports are submitted to the Youth Bureau and regular site inspections are conducted for evaluation.  For the past ten (10) consecutive years, Adelante has successfully complied with all the programmatic requirements of the program.

Housing & Tenant Counseling Services

The most pressing and immediate need is intervention on behalf of tenants to prevent homelessness and to assist those who are homeless. This includes assistance in finding affordable housing; advocacy and dispute resolution in landlord-tenant matters, assisting tenants in money management, and household budgeting. We also address the needs of tenants by referring them to programs such as HEAP, Section 8, Social Services, and Public Housing.  Assistance with applications to these programs will be provided, particularly for those families with language and cultural barriers.  Teaching low-income families household management and coping skills will lead to their becoming independent and self-reliant.

During the past fiscal year 2010, we have provided housing assistance as follows: (1) Conducted Household & Maintenance/Repair Workshops (2) Provided community residents with information on First Time Home ownership programs, assisted in the application process, reviewed credit status and made referrals for further assistance and, (3) Provided debt counseling and loan/grant workshops. We also provided Tenant Housing Assistance as follows: (1) Grant Subsidy application with renters, processed Section 8 recertification, Section 8 application; assisted clients with attaining Department of Social Services emergency housing, Domestic violence shelter and sober house placement; (2) Provided community residents with referrals to local agencies for financial assistance, Social Security recertification and initial application process, unemployment benefits and other related services; (3) Provided renters with information on tenant rights, advocate on behalf of tenant during illegal eviction process, review of lease, illegal entry by landlord to apartment, request for apartment repairs and relocation and housing court assistance.

The above activities have a major impact in reducing the incidence of homelessness in the community and will give tenants the needed resources and training to be more successful in managing their housing situations.

The program is evaluated and monitored by New York State Division for Housing and Community Renewal.

Nutrition Program For The Elderly

Adelante provides senior citizens in our community with the opportunity to enjoy nutritious and delicious meals five days a week at a very low cost of $2.00 a meal, which many are able to afford on limited income.  In addition to our nutritional benefits we also offer them the opportunity to socialize among their peers, participate in-group outings; attend legislative forums in Albany focusing on issues relating to seniors, provide recreational activities, guest speakers addressing issues and concerns of seniors such as; Health Medicaid/Medicare, Food Stamps, Heap, Counseling and Low-Income Housing needs.

Adelante also provides transportation services to seniors for their medical and dental appointments; pick up their prescription drugs, shopping for food and other essential needs, and to Family and Nutrition Services.

The Suffolk County Office for the Aging evaluates and monitors this program. Adelante has seen visible improvements on the part of the clients in regard to their willingness to participate and interact. A monthly report is being submitted to the Office for the Aging for evaluation and monitoring purposes. A representative makes regular site visits that cover safety and sanitation; and programmatic monitoring is also conducted. For the past ten (10) years, we have received favorable results.

Educational, Career Counseling And Youth Leadership

This program was brought to fruition in 1995 as a result of the increasing disparity in the technological proficiency between low and middle-income people.  It includes computer skills training, a program that started in 1994, which offers hands on training for those who are unemployed.  Also included is English as a Second Language, workshops in youth leadership, self-esteem, employment opportunities, and technology to help students meet the changing demands of today’s job market.

On May 17, 2006, IBM Corporation granted us a license to access and to use the IBM Reading Companion program and educational materials solely for instruction and learning to help benefit our youth/ students and consumers. Educational materials includes online books, online training, online evaluation materials including online tutorials and online instructor guides posted on Reading Companion website www.readingcompanion.org

On November 27, 2006, IBM Corporation granted us a license to use and access TraduceloAhora (Automatic Translation software). Email translation is now available to enhance the communication between parents and teachers as well as students, schools and other users affiliated with Adelante that are part of this grant program. A Hispanic student can email in Spanish a letter /messages and the recipient will receipt the letter / messages in English or vice versa.

Those who completed the training are invited to a graduation ceremony and are qualified to receive a used computer. There are 12 to 15 used computers raffled every year. Graduates of this program are more marketable in the job market; over 70% have found a good paying job and /or improved their current status of life.

Town of Islip – Community Development Administration (CDA) and Suffolk County Youth Bureau monitor this program every year.

After School Program

Adelante provides the After School Program at the following school districts:

  1. Amityville Union Free School District (Park Avenue Memorial Elementary and Edmund Miles Middle Schools). To develop and implement Family Clinics at each of two (2) middle schools; provide bilingual support services to school counselors for the purpose of community/parental outreach and provide bilingual support during the bi-monthly workshops. Provide Weekly Information, Resource and Referral Services to the families of participating students at each of the Centers.

    Days and Hour of Operations: Monday to Friday at 2:00 pm to 5:00 pm.
  1. Bay Shore Union Free School District (Middle School & Intermediate School). Adelante provides a weekly Family Center, focusing on developing computer, vocational and educational skills for parents/guardians and host bi-monthly workshops at each of the after school sites, focusing on parental needs in the following areas: (a) Department of Social Services, Social Security, Domestic Violence, Child and Elderly abuse, Immigration, Discrimination, Housing, Education issues, Veterans Affair and Crisis Intervention.

  2. Brentwood Union Free School District (North Middle School, East Middle School, West Middle School & South Middle School and High School) Adelante developed and implemented Family Clinics at each of the four middle schools and high school. The focus was on the development of weekly computer, vocational and educational skills for parents/guardians. In addition, bilingual case workers provided bi-monthly workshops focusing on parental needs in the following areas: (a) Department of Social Services, Social Security, Domestic Violence, Child and Elderly abuse, Immigration, Discrimination, Housing, Education Issues, Veterans Affairs and Crisis Intervention.

Medicaid Service Coordination Program

Adelante provides a Medicaid Service Coordinator (MSC) that helps individuals with developmental disabilities and mental retardation in exploring what they want and need in life, and assists them in gaining access to necessary supports and services. Our Service Coordinator works hand in hand with the consumer in the development and implementation of a person-centered plan to attain the consumer’s goals. The Service Coordinator assists the consumer in attaining the highest quality of life to live productively and as independently as possible. The MSC works with the consumer as their advocate by (i) Obtaining supporting information for planning, (ii) Identifying valued outcomes or personal goals, (iii) Planning and coordinating the supports and services the consumer needs and wants to live as he desires, (iv) Implementing the plan by linking and referring the consumer to activities, supports and services, (v) Overseeing the plan to the consumer’s satisfaction, and, (vi) Monthly face-to-face meetings and quarterly meetings at the individual’s home.
